I think the problem in this line: check process pulseaudio matching "pulseaudio"
Even when the pulseaudio is not running it says "running" so I guess it does not recognize it.

All of them "Execution failed"
Currently the Pulseaudio is running so the Monit should not do anything just recognize the running process.

The problem is you have the x11-pulseaudio package installed which is always running so the matching statement is finding that. Despite my efforts I'm unable to match the correct one.
You can try removing the x11-pulseaudio package to see if it helps and hope the regular pulseaudio package stays installed.
